Step 1
- Print out 1 admission form per family
Step 2
- Print out 1 church school enrollment form for each student
Step 3
- Fill out all forms, make sure to sign and date them
Step 4
- Send all forms with a check or money order made out to CUMCHA
Once we
receive all these you will get a packet from us and a letter stating
that you are enrolled.
If you
prefer you can enroll in person in our office. Please call ahead to
schedule an appointment or come by on Thursdays after 1:30pm.
*High
school students must have a transcript, it does not have to be
"official" just from the school.
